IRL, multi-barreled pistols frequently would discharge all barrels at once when only one barrel was intended to be fired. Some chance of that happening would be appropriate. However, off the top of my head I can think of no easy (fast) way of doing that so leaving it out may be best.

SW handles the issue pretty well, I agree on that. I had actually something similar to that before but I noticed that firearms became to powerful, so I changed it. But perhaps something along those lines could work in a slightly more modern setting, like the fantasy western mini-setting I am thinking about.

Personally, I like the Savage Worlds approach to ammo...at least for extras, you've got it until you roll a 1, at which point your ammo level drops one. There are 4 ammo levels, and you start at 3, when you hit 0 you're out...you can top it off again if you're in civilized regions, perhaps having to pay some money. So ammo occasionally comes up as an issue, you have to think about restocking sometimes, but there's hardly any ongoing bookkeeping.
